Request an Occurrence Summary

An occurrence summary is a very brief report which summarizes police involvement in an occurrence.  The type of request is generally made for insurance purposes.  It can be requested by the insurance company whose insured is directly involved, the law firm representing the client, or the person or company directly involved.  Each report costs $62.15 (includes HST).

These types of requests should be requested by submitting the fee and completing the Routine Disclosure Request/Consent form.  Consent of the insured or client is required at time of application.
